THE PROJECT "TELESURGERY AND TELETRAUMATOLOGY OF THE MAXILLO-FACIAL AREA, TELESTOMATOLOGY"

Nowadays the new branch of medicine is introduced to the practical activity of doctors of different specialities. It is telemedicine that gives the opportunity to use the new integrated informative and telecommunicative technologies in combination with doctors' experience for rendering highly qualified aid to patients in distant regions [1]. In most developed countries of Europe, the USA and Russia the centers of telemedicine have been founded and function successfully [2,3,4]. The telemedical systems have been introduced in Ukraine for 10years [5]. At the same time it should be noted that information as for usage of the telemedical procedures in the maxillo-facial surgery and stomatology is described rarely [6,7].
As a result of the given circumstances and being influenced by necessity of improvement of interaction between the separate links of the medical institutions of stomatological profile of different levels conditioned by the results of analysis of reasons of development of complications during traumatic injury and lethality during suppurative-inflammatory diseases of the maxillo-facial area which has been performed in the Clinic of the maxillo-facial surgery of the Donetsk State Medical University (DSMU) named after M.Gorky for 10-13 years [8,9], realization of the project "Telesurgery and Teletraumatology of the Maxillo-Facial Area, Telestomatology" within the framework of the project "Telemedicine in Ukraine" has been started since September of 2004 year.
The first step of realization of this project has been creation and successful usage of the multifunctional digital computer complex for receiving, processing, storage, transmission and visualization of information for 3 years on the basis of which the basic work station (BWS) functions [10]. The second step is the test control of this equipment, in particular- the notebook and mobile telephones with photo and video cameras, when the trial teleconsultations are performed during departure to the medical institutions of town for rendering consultative and medical aid with the help of the line of the Center of emergency medical aid. The third and basic step is creation of the specialized polythematic official site of the chair of the surgical stomatology with the clinic of the maxillo-facial surgery DSMU in Internet in two versions- Russian (pic.1) and English (pic.2).
The site is dedicated to the questions of the maxillo-facial surgery and stomatology. Information about the history of the chair of surgical stomatology with the clinic of the maxillo-facial surgery DSMU, its collaborators, achievements and success is presented in the parts of the site. The volume of the consultative and medical aid rendered in the chair and in the clinic, the new original elaborations and the scientific researches are presented. Students have the possibilities to get the necessary information for preparation to classes and examinations. One of the directions of the site is publication of the materials on the actual questions of the maxillo-facial surgery and stomatology, acquaintance of our colleagues with the new methods of patients' treatment with different pathology of the maxillo-facial area. The references on the interesting sites dedicated to the questions of medicine and contiguous with it domains of human activity are presented separately.
The separate part of the site dedicated to teleconsultation - direct realization of the project "Telesurgery and Teletraumatology of the Maxillo-Facial Area, Telestomatology" (pic.3). This project presupposes performing of the following telemedical procedures - asynchronic and synchronic teleconsultation, telenursing, teledispensary system and distant studying.
Information for doctors (pic.4) and patients (pic.5) is presented separately. The form of free consultation (second opinion) of the specialists of the chair and the clinic (pic.6) has been elaborated for patients and placed in the site, the form of the inquirer (adviser, coordinator) (pic.7) and the form of doctor's inquiry for teleconsultation (pic.8) have been created for doctors. The indicated forms give the opportunity to draw up and send the inquiry for teleconsultation quickly and easy placing in the margins the information necessary for an adviser. If an inquirer can't use these forms because of any reasons the inquiry for teleconsultation can be sent to the e-mail. The necessary demands for drawing up the inquiry and enclosed text and graphic files (pic.9) are presented in the site for such cases.
The collaborators of our clinic in cooperation with the collaborators of the department of informatics and telemedicine of the Scientific Research Institute of Traumatology and Orthopedics of the Donetsk State Medical University named after M.Gorky and also the Donbass Institute of Technology and Management of the International Technical University (DITMITU) (Kramatorsk) take part in realization of the project "Telerehabilitation (Telenursing + Distant studying)" (pic.10) that presupposes telenursing of invalids with different diseases who take the cycle of distant studying in DITMITU.
At this moment the project "Teledispensary system" is at the stage of elaboration that presupposes the regular medical check-up of patients with different diseases of the maxillo-facial area as in the stages of preoperative preparation so during postoperative regimen management of patients in the outpatient setting using the standardized card of the regular medical check-up and telecommunicative technologies.
We have performed 9 teleconsultations during the first 3 months since realization of the project "Telesurgery and Teletraumatology of the Maxillo-Facial Area, Telestomatology". 3 of them are in the interregional level; one is in the state one and 5 of them in the international level (Russia, Lebanon). 6 teleconsultations have been performed with usage of e-mail, 3 of them -with usage of means of mobile communication (SMS, MMS). 4 teleconsultations have been performed to confirm a diagnosis and policy of treatment of the inflammatory diseases of the maxillo-facial area, 3- the combined traumas with injury of bones and the soft tissues of face, 1- tumors of the parotid gland, 1-planning of the plastic operation. The performed teleconsultations gave the possibility to confirm the diagnosis and define policy of treatment of patients urgently.
The small quantity of consultations testifies that telemedicine in the maxillo-facial surgery in Ukraine makes its first steps (the fact that 5 from 9 consultations on the inquiries are from abroad is confirmation o it), there are all the grounds for the further development of this branch of medicine and introduction of it into practice of the medical prophylactic institutions of stomatological profile of all the levels. It is absolutely natural this problem can't be decided only at the expense of activity of collaborators of our clinic, that's why the quickest introduction and development of telemedicine in the maxillo-facial surgery and stomatology is seen in cooperation with the other leading maxillo-facial and stomatological clinics and also with the Centers of telemedicine in different regions of Ukraine, in the other foreign countries.

РЕЗЮМЕ
Описаны первые этапы осуществления проекта "Телехирургия и Телетравматология челюстно-лицевой области, Телестоматология" в рамках проекта "Телемедицина в Украине". Приведен первый положительный опыт оказания консультативной помощи с использованием возможностей данного проекта при проведении 9 телеконсультаций по различным вопросам челюстно-лицевой хирургии и стоматологии. Намечены перспективы дальнейшего развития и совершенствования проекта

Приведений перший позитивний досвід надання консультативною допомоги з використанням можливостей даного проекту при проведенні 9 телеконсультацій з різних питань щелепно-лицьової хірургії і стоматології. Намічені перспективи подальшого розвитку і вдосконалення проекту.

SUMMARY
The first stages of the project "Telesurgery and Teletraumatology of the Maxillo-Facial Area, Telestomatology" within the framework of the project "Telemedicine in Ukraine" are described. The first positive experience of rendering the consultative aid using the possibilities of this project is given when 9 teleconsultations on different questions the maxillo-facial surgery and stomatology were performed. The prospects of the further development and improvement of the project are outlined.
